All student athletes are required to have an up-to-date physical that was done within 12 months of first practice and it needs to be submitted to the school Health Office.  Also, an “Interval Health History for Sports Participation” is required for EVERY sport season.  From now on, the "Interval Health History" will be turned in through FamilyID and we will no longer accept the paper copy.  Since this form cannot be submitted too early, the online system will be open for registration exactly 30 days before first practice.

If your child is enrolled in the Morris Health Zone, please call 263-2619 to schedule your child’s sports physical.  If you will be using your private provider, please mail the physical to:

Please note that there are new Health Examination Guidelines for Schools effective 02/01/2021.  We can no longer accept your private physician's annual health physical forms.  All health physicals must be filled out on the NYS Required Health Examination form.
