Medication Administration in School
As per New York State guidelines, the following requirements are necessary for medication administration in school:
- All medications must be delivered to school by a parent/guardian, unless a self-carry order form is signed by both the parent and the child's physician.
- All medications must remain in the original and properly labeled containers (including all over-the-counter medications).
- A new provider's order/medication administration form, signed by both the medical provider and the parent/guardian, is required at the beginning of each school year.
- All provider's orders/medication administration forms must be filled out completely by both the parent/guardian and the child's provider. These forms will be considered null and void, if not fully completed. The school is not responsible for obtaining medication orders from your child's provider; it is the responsibility of the parent/guardian.
For your convenience, the medication administration form (Authorization for Administration of Medication) is posted below. Please print and fill out the form completely before bringing any of your child's medications to school. We will also accept medication order forms from your child's provider's office as long as they are fully completed, signed by the provider and signed by the parent/guardian.
End Of The Year Medication Pick-Up
Any medications that remain in the Nurse's Office at the end of the school year need to be picked up by a parent/guardian. You can arrange the pick up of medications with the school nurse by calling (607) 263-6111. Please be aware that any medications not picked up by 3:00 pm on Friday, June 23, 2023 will be disposed of.
